Medicare Primary
If Medicare is the primary payer, CFKC will bill Medicare directly. MEDICARE ASSIGNMENT IS ACCEPTED. Annual deductibles and the remaining 20% not covered by Medicare for treatments, medications and other services outisde the bundle are the responsibility of the patient. CFKC will bill a secondary payer for patients staying over one month.
Patients dialyzing less than one month will be required to pay any deductibles the estimated 20% and othe charges outside the bundel in advance.
Insurance Primary
Patients staying less than one month will be requested to pay in advance for treatments, medications, anticipated lab and other services. An itemized statement will be provided at the end of the month for insurance purposes.
Insurance Secondary
CFKC will accept FLORIDA Medicaid and FLORIDA Blue Cross secondary insurance through crossover claims processing.
Veterans Administration
CFKC will bill and be directly reimbursed by the Veterans Administration. An authorization form for all services must be provided in advance.
Foreign Patients
CFKC requires payment in advance for treatments, medications, anticipated lab and other services. An itemized statement will be provided at the end of the month for insurance purposes.
Seasonal Patients
Individualized consideration regarding advance payments is given to patients staying over one month. Contact us for details.

Cancellation for Dialysis Treatment
A 24-hour cancellation notice is required for full refund of prepayment.
A $50.00 supply charge will be assessed for hemodialysis treatments missed without 24-hour advanced notice.

Central Florida Kidney Centers, Inc is a not-for-profit facility and is governed by a Board of Directors consisting of lay people who are interested citizens, and leaders in the community.
